  • Payment Slip / Certificate of Insurance
  • Supplementary documents to support the claim

FOR LOSS CLAIM
  • Proper Identifications
  • Payment Slip / Certificate of Insurance
  • Police Report &/or excerpt from the blotter (must be reported within 24 hours from the time of loss)
  • Supplementary documents to support the claim
  • Accomplished Claims Report Form (file within 48 hours after loss)
  • Proof of ownership
